It sounds like a broken X axis encoder wire. These wires are fitted in a plastic energy chain for support while the axis is moving. You need to locate the X axis encoder at the rear of the X axis motor via the door on the right hand side of the machine.

There should be a round plug on the encoder unit (on the end of the X axis motor), with the power OFF to the machine, disconnect this plug. Then open the control box and locate a 20 way Honda plug (Marked M35) see sheet 26. Using an electrical meter ensure that the continuity of each wire is intact

This may also help:

Fanuc axis Diagnostic no 720-723 are axis error bits - one for each axis up to 4:
bit 0 (right hand side): overflow alarm (OFAL)
1: wire disconnection alarm (FBAL)
2: regen discharge unit alarm (DCAL)
3: overvoltage alarm (HVAL)
4: an improper current alarm (HCAL)
5: excessive current alarm (OVC)
6: voltage shortage alarm (LV)
7: overload alarm (OVL)

BTW, I thought the X axis error would be 406 ?
4x6 is the error type and x is:
Milling:
0 for X axis
1 for Y axis
2 for Z axis
Turning:
0 for X axis
1 for Z axis
